Badlands Pet Crematory

 

By submission of payment I authorize that I am the owner of said pet and have the right to give permission to Badlands Pet Crematory of Dickinson, ND, to take possession and care of and complete the irreversible cremation process for my pet. Due to the nature of the process of cremation, any materials on the pet such as collars or blankets will be destroyed unless removed.  Once cremated, pets are placed in a temporary plastic urn. I release and hold harmless Badlands Pet Crematory, their employees and affiliates, from any and all liabilities, claims or expenses arising from and related to the handling, cremation, and release of my pet.
